The version of iOS or iPadOS on your computer. Open the iOS/iPadOS Settings. Go to the General section and choose About.
Your actions that could have led to the problem.
If the problem persists, describe how to reproduce it.
When necessary, illustrate your report with screenshots (press and hold the Sleep/Wake button and immediately press the Home button).
